Youth Challenge of CT, Inc. Second Phase Transitional Center
Transitional Substance Use, Mental Health and Case Management Services are provided to Men who are 18 years old and older. These services are provided at 17 May Street Hartford, Connecticut. Services provided in our Second Phase Transitional Center include 4 hours a week of group and individual case management services. These services are provided in a residential setting that provides a safe and structured environment. The groups provided are more recovery and relapse prevention based as the men in the transitional program are now connected with out-patient based community substance use and mental health treatment services. Case Management services are also provided in order to assist the men in the program with educational, vocational and housing referral services in order to the men to re-enter their community or return to their own family environment. Family Therapy services are also available in order to ensure a healthy and beneficial return of the men back to their families.
